Mortality and morbidity depend on the grade and stage of tumour at the time of diagnosis. Early diagnosis is associated with good prognosis, but there are limited data in Bangladesh about such practice.

Objective: The aim of the study is to see the histomorphological pattern of urinary bladder tumors in different age groups and to see the pattern of presentation of various urinary bladder tumours.

Methodology: A total 150 cases, clinically, sonographically and/or cystoscopically suggestive of urinary bladder tumor were included in this study. Specimens were collected after surgical resection in a container containing 10% formalin. The specimens were processed by paraffin embedding method and stained by routine Haematoxylin and Eosin stain for microscopic examination. The results of this study were calculated by standard statistical formula.

Results: This study focuses mostly on the histological examination of the specimen of urinary bladder mass with their clinical findings. The microscopic type of 150 cases of urinary bladder tumor was: 143 (95.33%) were papillary urothelial carcinoma, 4 (2.66%) were adenocarcinoma, 2 (1.33%) were squamous cell carcinoma and I (66%) was carcinosarcoma. The main clinical presentations were haematuria, irritative voiding of urine and flank pain.

Conclusion: Papillary urothelial carcinoma stage T2 are much more common in Bangladesh. Regarding association with risk factor, smoking plays very important part for making bladder tumour.

Bang Med J Khulna 2021; 54 : 03-07
Md Mahabubur Roshed, Masud Ahmed MS, Mohammad Ali Copyright (c) 2021 Md Mahabubur Roshed, Masud Ahmed MS, Mohammad Ali https://banglajol.info/index.php/BMJK/article/view/60776 Mon, 18 Jul 2022 00:00:00 +0000 Association of lipid profile with severity of hepatitis B virus related chronic liver disease in adult https://banglajol.info/index.php/BMJK/article/view/60777
Background: Chronic liver disease (CLD) is a major health problem and contributes to a common cause of hepatocellular carcinoma, disability and death worldwide. Hepatitis B virus (HBV) is the leading cause of CLD in our country. Abnormal lipid profile is expected in those with severe liver dysfunction.

Objective: The objective of the study was to determine the alteration of lipid profile (total cholesterol, triglyceride, LDL-C and HDL-C) in HBV positive adult CLD patients and to correlate with severity of liver damage.

Methods: This case control study was conducted for one year between January 2018 to December 2018. One hundred and three patients, >18 years old, admitted with HBV positive CLD in Dhaka Medical College Hospital were recorded as case and 103 age and sex matched, healthy population, were recorded as control. A standard case record form including demographics, histories, clinical examinations and different investigations were completed for each group. Then lipid profile values of cases were compared to controls and different Child-Pugh and MELD scores, were used to see severity of liver damage in CLD patients.

Results: In patients with HBV positive adult CLD, there were a significant decrease in serum total cholesterol. LDL-C and HDL-C levels compared to the control group (mean 131.4 vs 179.3, 72.5 vs 109.6, and 31.0 vs 40.4 mg/dl, respectively; p<0.001). Comparison of lipid profile with pathologic progression of CLD revealed that these values were diminished gradually with progression of liver damage. Overall triglyceride level was decreased marginally in cases when compared to controls (mean 139.2 vs 146.6 and p=0.047) and this reduction was observed in only advanced stage.

Conclusion: Serum total cholesterol, LDL-C and HDL-C level in HBV positive CLD patients were low and diminished gradually with severity of liver damage. Triglyceride level was marginally significant and diminished in advanced CLD.

Bang Med J Khulna 2021; 54 : 08-12
Md Asiful Islam, Hamid Hasan, Gulnar Yasmin, Minhaj Rashidur Rahman, Md Titu Miah Copyright (c) 2021 Md Asiful Islam, Hamid Hasan, Gulnar Yasmin, Minhaj Rashidur Rahman, Md Titu Miah https://banglajol.info/index.php/BMJK/article/view/60777 Mon, 18 Jul 2022 00:00:00 +0000 Effectiveness of nitazoxanide in the treatment of COVID-19 related diarrhoea https://banglajol.info/index.php/BMJK/article/view/60778
Background: Diarrhoea is becoming one of the most prominent and troublesome manifestations in COVID-19 infection but no definitive treatment is available. So Nitazoxanide was used to see whether it can shorten the duration and sufferings form COVID-19 related diarrhoea.

Objective: To see the efficacy of Nitazoxanide in decreasing frequency and improving the consistency of stool in COVID-19 related diarrhoea.

Methods: This randomized comparative study was done in private chambers of medicine specialists in Khulna and patients admitted in corona dedicated hospital, Khulna from July 2020 to June 2021. 100 patients were enrolled in the study using Nitazoxanide & ORS (oral rehydration solution) in 50 patients and only ORS in another 50 patients. Decreased frequency and volume of stool at 24, 48 and 72 hours of both groups were compared.

Results: After giving Nitazoxanide in diarrhoea of COVID-19 patients, improvement was in 90% patients at 24 hours: in 96% patients in 48 hours and in 98% cases at 72 hours. On the other hand, without Nitazoxanide improvement was found in 30%, 40% and 50% cases at 24, 48 and 72 hours respectively. So Nitazoxanide was effective (p <0.05) at reducing frequency of diarrhoea and improving the consistency of stool.

Conclusion: Nitazoxanide significantly improves diarrhoea in COVID-19 patients by reducing duration of diarrhoea and improving consistency of stool.

Bang Med J Khulna 2021; 54 : 13-16
Md Nazrul Islam, Sabikun Nahar Chowdhury, Md Monir Uz Zaman, Md Zahirul Huq Copyright (c) 2021 Md Nazrul Islam, Sabikun Nahar Chowdhury, Md Monir Uz Zaman, Md Zahirul Huq https://banglajol.info/index.php/BMJK/article/view/60778 Mon, 18 Jul 2022 00:00:00 +0000 Comparison of back muscle and leg muscle strength among different groups of female medical students https://banglajol.info/index.php/BMJK/article/view/60779
Background: Muscle strength is the ability of a muscle or muscle group to exert force. Good muscle strength increases physical performance and may be important in weight maintenance.

Objective: The present study aimed at comparing the back muscle and leg muscle strength among different groups of female medical students which might be used as base-line findings for other similar sedentary professionals.

Methods: A cross-sectional analytical study was conducted in the department of Anatomy, Dhaka Medical College, Dhaka, from January 2016 to December 2016. The mean back muscle and leg muscle strength were higher in normal group than overweight group and lower in obese group. Significant difference (p=0.030) was observed between mean leg muscle strength of over-weight and obese group.

Conclusion: The results of the present study demonstrate that back muscle and leg muscle strength were significantly higher in normal group than overweight group and it is lower in obese group.

Bang Med J Khulna 2021; 54 : 17-20
Samina Azad, Md Mohibul Hasan Khan, Shamima Hoque Lia, Shamim Ara Copyright (c) 2021 Samina Azad, Md Mohibul Hasan Khan, Shamima Hoque Lia, Shamim Ara https://banglajol.info/index.php/BMJK/article/view/60779 Mon, 18 Jul 2022 00:00:00 +0000 Implication of serum hepcidin level in maintainance haemodialysis patients in association with different iron indices https://banglajol.info/index.php/BMJK/article/view/60781
Background: Anaemia is a frequent complication in maintenance hemodialysis (MHD) patients and is associated with increased morbidity and mortality. Altered systemic iron metabolism and functional iron deficiency mainly related to subclinical inflammation makes it difficult to maintain proper control of anaemia. The discovery of hepcidin and it's functions has contributed to a better understanding of iron metabolism disorders in CKD anaemia.

Objective: To evaluate the association of serum hepcidin with different iron indices (serum iron, serum ferritin, TIBC and TSAT).

Materials and Methods: The study was carried out in the Institute of Kidney Diseases and Urology, Dhaka for a period of six months between July to December 2016. Serum hepcidin-25, current iron markers and other laboratory parameters were measured among study subjects. Absolute and functional iron deficiency patients were identified according to serum ferritin and TSAT. Statistical analysis was performed to find out whether serum hepcidin level significantly differ in MHD patients than healthy controls and also to evaluate it's correlation with other iron indices among MHD patients.

Results: Total 88 subjects (fifty MHD patients and thirty eight healthy controls) were enrolled into the study. Serum hepcidin level was significantly higher in MHD patients than healthy controls (19.3 (7.0 - 81.8) ng/ml vs 8.0 (2.4 - 33.6) ng/ml, P value <0.001). Hepcidin has significant positive correlation with ferritin in MHD patients (r=0.480; p=<0.001). Hepcidin was also positively correlated with serum ferritin both in absolute iron deficiency patients (r=0.786; p=0.036) and functional iron deficiency patients (r=0.764; p=0.006).

Conclusion: Serum hepcidin level is increased in MHD patients and associated with disturbance of iron metabolism. Hepcidin positively correlates with serum ferritin in MHD patients and may be used similarly as ferritin in guiding iron therapy.

Bang Med J Khulna 2021; 54 : 21-25
Palash Tarafder, Md Ayub Ali Choudhury, Md Babrul Alam, Tofael Ahammod, Pritish Tarafder Copyright (c) 2021 Palash Tarafder, Md Ayub Ali Choudhury, Md Babrul Alam, Tofael Ahammod, Pritish Tarafder https://banglajol.info/index.php/BMJK/article/view/60781 Mon, 18 Jul 2022 00:00:00 +0000 Giant aneurysm of the splenic artery https://banglajol.info/index.php/BMJK/article/view/60782
Splenic artery aneurysms (SAA) are one of the common visceral aneurysms. However giant SAA have barely been reported. We discuss a case of SAA, who had prior history of caroli disease at the stage of secondary biliary cirrhosis for which he was a candidate for liver transplantation. The patient underwent open surgical repair. The proximal and distal spenic artery were ligated from within the aneurysm and surgery was completed with splenectomy. The post operative period was uneventful.

Bang Med J Khulna 2021; 54 : 26-27
Rania Osmane, Firas Ayadi, Douha Bacha, Ferjaoui Wael Copyright (c) 2021 Rania Osmane, Firas Ayadi, Douha Bacha, Ferjaoui Wael https://banglajol.info/index.php/BMJK/article/view/60782 Mon, 18 Jul 2022 00:00:00 +0000 Abstract From Current Literature Vol. 54 (1-2) https://banglajol.info/index.php/BMJK/article/view/60783
Abstract not available

Bang Med J Khulna 2021; 54 : 28-31
Choudhury Habibir Rasul Copyright (c) 2021 Choudhury Habibir Rasul https://banglajol.info/index.php/BMJK/article/view/60783 Mon, 18 Jul 2022 00:00:00 +0000 The multisectoral approach to prevent next pandemic https://banglajol.info/index.php/BMJK/article/view/60775
Abstract not available

Bang Med J Khulna 2021; 54 : 01-02
CH Rasul Copyright (c) 2021 CH Rasul https://banglajol.info/index.php/BMJK/article/view/60775 Mon, 18 Jul 2022 00:00:00 +0000
