Thyroid disorders in pregnancy: Evidence-based management from preconception to postpartum

In women of childbearing age, thyroid disease is a common endocrine disorder. Wrong choice of treatment, as well as delayed commencement and adjustment of treatment, can result in detrimental effects on pregnancy and the fetus. Namely, pre-eclampsia, placental abruption, gestational hypertension, postpartum hemorrhage, miscarriage or stillbirth, preterm delivery, low birth weight, fetal growth restriction, respiratory distress syndrome, and neurodevelopmental deficits. Therefore, it is very important to optimize thyroid care when planning pregnancy, during pregnancy and after birth. Substantial new clinical and scientific evidence is now available in the American Thyroid Association 2026 guidelines and RCOG 2025 guidelines to enable healthcare professionals to better manage thyroid disorders in pregnancy and also before and after pregnancy.
