TVS and MRI revealed well defined solid mass in retroperitoneal at the level of L5 to S2 vertebrae and displace right common iliac and external iliac vessels. Limited cuts of the region of Computed tomography (CT) were taken and no calcification was identified. We are reporting a case of 16 years old male student presented with acute febrile illness with icterus, hemorrhagic manifestation and pulmonary-renal involvement, diagnosed as Severe Leptospirosis (Weil's disease) confirmed by ICT method. To protect against sepsis prompt recognition, careful hemodynamic support, appropriate antibiotics, and control of the source of infection are required. With the trend in management, one should understand sepsis physiology and follow the best practice guidelines. Sepsis and particularly septic shock should be recognized as medical emergency as in stroke and acute myocardial infarction. Early recognition and rapid institution of resuscitative measures are critical. Although recognition of sepsis can be a challenge and the best management practices continue to evolve.
