Incidentally Detected Rare Retroperitoneal Tumor: Ganglioneuroma

A primary extra-adrenal retroperitoneal ganglioneuroma was found incidentally in a 22-year-old woman during screening for suspected pregnancy by transvaginal ultrasonography (TVS) and subsequently by magnetic resonance imaging (MRI). TVS and MRI revealed well defined solid mass in retroperitoneal at the level of L5 to S2 vertebrae and displace right common iliac and external iliac vessels. Limited cuts of the region of Computed tomography (CT) were taken and no calcification was identified. Histological examination showed that the lesion was an extra-adrenal retroperitoneal ganglioneuroma composed of mature ganglion cells and subsequently confirmed by immunohistochemistry.

Intranodal Palisaded Myofibroblastoma: A Case Report

Intranodal palisaded myofibroblastoma is a rare benign soft tissue tumor almost always arising from inguinal lymphnodes. It usually present as a painless, slow growing inguinal mass. We report herein a case of an intranodal palisaded myofibroblastoma occurring in a 43-year-old man along with its salient clinicopathologic features and differential diagnosis.

A Young Boy with Severe Leptospirosis (Weil's Disease): A Case Report

Leptospirosis, a zoonotic disease is increasingly being recognized in developing countries. It is often underdiagnosed resulting in significant mortality as the presenting features mimic other commonly prevailing diseases like Malaria, Dengue, Acute hepatitis, Typhoid in developing world. We are reporting a case of 16 years old male student presented with acute febrile illness with icterus, hemorrhagic manifestation and pulmonary-renal involvement, diagnosed as Severe Leptospirosis (Weil's disease) confirmed by ICT method. This experience highlights the importance of considering Leptospirosis early in the diagnosis of acute nonspecific febrile illness with multi-organ involvement to cutoff mortality from this fatal disease.

Rupture of the ventricular septum, a rare but potentially fatal complication of acute myocardial infarction, is usually observed in the setting of acute myocardial infarction associated with major coronary artery occlusion. It is often accompanied by severe circulatory failure. In this paper we describe a case of a 70-year-old lady who presented with cardiogenic shock 7 days after acute myocardial infarction and was found to have ventricular septal rupture.

It can represent a diagnostic challenge because it may present in various and unexpected ways. We report the case of a 65-year-old male who sustained shotgun pellet emboli to left ulnar artery following a right and left upper extremity gunshot injury.

Stress - An overview

Background: Stress can be described as a circumstance that disturbs or is likely to disturb the normal physiological or psychological functioning of a person. It may seem that there's nothing one can do about stress. But in fact the control can be more than one can think about.

Objective: Create awareness about psychological support in work-life balance, reduced stress levels at work and at home, greater focus and concentration, participate more fully in family and social life, pursue personal goals and hobbies, improved health.

Conclusion: The simple realization that one can be in control of his/her life is the foundation of stress management.

Sepsis: Updates

Sepsis is a life-threatening organ dysfunction that results from the body's response to infection. To protect against sepsis prompt recognition, careful hemodynamic support, appropriate antibiotics, and control of the source of infection are required. With the trend in management, one should understand sepsis physiology and follow the best practice guidelines. Sepsis and particularly septic shock should be recognized as medical emergency as in stroke and acute myocardial infarction. Early recognition and rapid institution of resuscitative measures are critical. Although recognition of sepsis can be a challenge and the best management practices continue to evolve. This article reviews guidance on the diagnosis and management of sepsis and septic shock with attention to maximizing adherence to best practice and controversies in definition, diagnostic criteria, and management.
